36-year-old woman with nodules on the cheeks.
The biopsy showed a paniculytis, predominatly lobular ( Figure 1).
The inflamatory infiltrate was granulomatous and epithelioid, with foreign-body giant cell reaction ( Figure 2, figure 3, figure 4).
The giant cells encircled round empty spaces that were not birrefringent under polarized light ( Figure 5, Figure 6, Figure 7, Figure 8, Figure 9).
These spaces were not uniform, varying in size.
When the condenser was lowered, no content was evidenced in the spaces. Some of the smaller spaces were totally included in the cytoplasm of the histiocytes, and many had a multilobated appearance, sometimes with a daisy-like aspect.
In many of the giant cells, asteroid bodies were found.
Histochemical techniques of Peryodic Acid of Schiff, Ziehl-Neelsen and Giemsa, did not show any micro-organism. A moderate amount of lymphocytes was also present in the infiltrate.
